Our school is partnering up with Smile New York Outreach. They will be providing dental checkup for our students, beginning January 24th through January 27th. If you would like to have your child seen, please fill out the dental form that has been sent home with your child.

Please return this form by Wednesday, January 4, 2023. If I do not receive the dental form back, your child will not be seen. You also have the opportunity to fill out the form online at www.myschooldentist.com
